Please note: Forms are at the bottom of the page Currently, all animal care and use applications and all animal care and use modification (amendment) forms must be submitted as Word documents to ACC Office via email (ooacc@uchc.edu). No PDF files can be accepted. Submissions must be electronic. Forms are below. We are working on electronic, web-based submissions via InfoEd- our database system that we use for management of the laboratory animal protocols at the Health Center, but we are not quite there yet. PLEASE DO NOT SEND SUBMISSIONS TO MULTIPLE MAILBOXES. PLEASE ONLY SEND SUBMISSIONS TO THE ACC OFFICE (e.g., do not send submissions to the veterinarian, ACC chair, and/or ACC coordinator as a submission to the ACC). All animal users must have current UCHC animal use training. You can sign up for training by going to the training event schedule and following the directions posted there. Protocols will not be approved unless all individuals listed as using animals have had UCHC animal use training. The ACC office has a workbook entitled "Working with the IACUC: Writing an Animal Protocol" which was written by the American Association for Laboratory Animal Science (AAALAS). It is a good resource for individuals who must write animal care and use protocols. You can get a copy by coming down to the office or contacting the ACC office at ooacc@uchc.edu. Deadlines for submissions are listed below: Note: Typically, deadlines are somewhat flexible. Please contact the ACC office at ooacc@uchc.edu if it is imperative that your submission be on a particular agenda date and you are submitting after the deadline date and time. The only exception to this is the deadline for the December genda- the deadline is firm at noon on 12/10/07.
OCCUPATIONAL HEALTH / MEDICAL SURVEILLANCE PROGRAM REQUIREMENTS: It is an institutional policy to have all individuals who use laboratory animals enrolled in the occupational health / medical surveillance program. This is needed to comply with PHS Policy on Humane Care and Use of Laboratory Animals. When people are added to a protocol, when a new protocol is submitted, and when annual reviews are due, the PI on the protocol will receive notification that all individuals on the protocol must enroll in the occupational health medical surveillance program.
IT IS THE RESPONSIBILITY OF THE PRINCIPAL INVESTIGATOR TO ENSURE THAT THIS ENROLLMENT IS ACCOMPLISHED BY ALL MEMBERS OF THE LABORATORY. PROTOCOL APPROVALS, ANNUAL RENEWAL APPROVALS, AND APPROVALS TO ADD THE INDIVIDUALS TO THE APPROVED PROTOCOLS WILL NOT OCCUR UNTIL ALL ENROLLMENTS REQUIRED ARE COMPLETED AND THE ACC HAS RECEIVED DOCUMENTATION FROM EMPLOYEE HEALTH SERVICES.
